Plan Options
We offer six distinct plans, each with its own biweekly premium and coverage limit:

• Non Referral: $11.10 biweekly with up to $25,000 in coverage
• Basic: $21.10 biweekly with up to $50,000 in coverage
• Basic Plus: $36.07 biweekly with up to $75,000 in coverage
• RetiCare: $29.54 biweekly with up to $75,000 in coverage
• Supplement: $59.11 biweekly with up to $100,000 in coverage
• Supplement Plus: $115.22 biweekly with up to $150,000 in coverage
  The premium split is structured as follows:

Employer Share: No less than 50%
Employee Share: No more Than 50%
Individuals: Responsible for 100% of the premium
